Wellness Program

Epsilon Systems includes a medical premium discount as part of our long-term benefits strategy to keep your health insurance costs as low as possible. You can lock in your Wellness Rate for 2027 by completing a few steps before October 31, 2026.

Important Details

  • Complete your wellness activity and submit your wellness form by October 31, 2026.
  • At Open Enrollment, you will see the wellness rate on your medical plan if you completed the requirements in time.

Requirements

Qualified Wellness Activities:

  • Annual wellness exam or no cost preventive visit with your primary care provider.
  • Annual lab work or Biometric Screening through your provider or at a clinic, including a lipid panel (cholesterol and triglycerides) and glucose/A1c.
  • Cancer screenings as recommended by your age and/or gender (e.g. mammogram, colonoscopy)

To submit proof of the exam, you have 3 options:

  1. Have your provider complete the Wellness Form
  2. Provide your Explanation of Benefits (EOB) from your visit
  3. Submit an After Visit Summary, making sure to redact personal health information

Wellness Program for New Hires

New hires can earn a 15% discount on medical plan premiums by completing one approved wellness activity within 90 days of their benefits start date. Options include preventive care visits, screenings, or an approved non‑medical alternative. Once verified, the discount starts at the next quarterly payroll date and can continue through the current and following plan year. Employee Assistance Program
