Medical Plan Network

Cigna PPO Network

The Cigna PPO Network is available for all plans. Although PPO appears in the name of the network, it applies to the HMO, HDHP, and PPO plan types and does not change the network coverage of each. This network offers a broad range of healthcare providers and facilities nationwide, giving you the flexibility to choose the care that’s right for you.

Health Plans, Inc. (HPI) Medical Plans

Selecting the right medical plan is key to supporting your health and your family’s. Here’s a quick side-by-side comparison of the HMO, PPO, and HSA, highlighting differences in costs, coverage, and flexibility to help you choose the plan that works best for you.

$500 Deductible HMO

Cigna PPO Network

This plan offers a $500 individual and $1,000 family deductible. With this plan, you will pay mid-level premiums and only in-network providers are covered.

$500 Deductible PPO

Cigna PPO Network

This plan includes a $500 individual and $1,000 family deductible. This plan will have higher premiums, but you will have the broadest network with the flexibility to see providers throughout the country.

$2,800 Deductible HDHP-HSA

Cigna PPO Network

This plan includes a $2,800 individual and $3,400 family deductible. This plan has the lowest monthly premiums and allows you to open a Health Savings Account (HSA) for tax-favored healthcare savings.

TRICARE Supplements from Selman

TRICARE Supplement Insurance helps reduce out‑of‑pocket costs by covering expenses left after your primary TRICARE plan pays. It wraps around TRICARE Prime, Select, or Retired Reserve coverage and helps pay deductibles, copays, cost shares, and certain excess charges.

Virtual urgent care and mental health through Doctor on Demand offers a convenient alternative to traditional in-person care. If you are enrolled in one of the HPI medical plans, you have 24/7 access to doctors and psychiatrists with Doctor on Demand. Copays vary by plan. Healthcare can be confusing, but you don’t have to navigate your care alone. Pathways Concierge is here to make your healthcare journey as easy as possible. Kidney Care – Specialty Care Management

For employees managing chronic or complex conditions, Specialty Care Management provides personalized nurse coaching and one-on-one guidance — at no cost to you. This care management program helps you get on track or stay on track with your kidney health or dialysis. A specialty Nurse Coach will help coordinate with your providers to make sure you are getting the best kidney care possible.

Green Imaging

If your doctor orders a medical imaging scan, such as an MRI, PET/CT, or X-Ray, Green Imaging connects you with high-quality providers at no cost or reduced cost when using a participating facility. If your doctor orders a diagnostic test, call, text, email, chat, or fax Green Imaging to see if you can save money on your test. You will need to send your name, your zip code, a picture of your physician order and your Group Name: Epsilon Systems Solutions. Sun Life – Dental PPO Plan

With the Dental PPO Plan through Sun Life, you have the flexibility to see any dentist, with greater savings when you stay in-network. Preventive services are included at no cost.

  • Oral exams and cleanings are covered 100%

  • $50 individual deductible and $150 family deductible

  • $2,000 calendar year maximum per member

  • Save the most when using in-network providers

Sun Life Vision Plan with the VSP Network

Epsilon Systems offers vision coverage through Sun Life with the VSP network to help you and your family maintain healthy eyes and save on the cost of exams, glasses, and contact lenses.

  • $10 copay for in-network eye exams and $25 copay for lenses every 12 months

  • $150 allowance + 20% off balance for frames every 24 months or

  • $150 allowance for contact lenses every 12 months

Health Savings Account (HSA)

A Health Savings Account (HSA) from Health Equity can be paired with our high-deductible health plan.

  • This account allows you to set aside pre-tax funds for future qualified healthcare expenses

  • Any unused funds in your account rollover year after year

  • Epsilon contributes $900 for individuals or $1,500 for families

Flexible Spending Account (FSA)

A Flexible Spending Account (FSA) through Igoe lets you set aside pre-tax dollars for qualified expenses. FSA funds are use it or lose it and must be spent by the end of the year calendar year.

  • Healthcare FSA – covers eligible medical, dental, and vision expenses (available only with HMO or PPO plan enrollment).

  • Dependent Care FSA – covers eligible expenses like daycare (available to all employees regardless of medical plan).

  • Limited Purpose FSA – For those enrolled in the HDHP plan (for eligible dental and vision expenses only).

Life and AD&D

Life and AD&D insurance provides financial protection by offering benefits in the event of death or dismemberment. Basic Life and AD&D is paid 100% by Epsilon and includes 1x your annual salary up to a maximum of $150,000. You may also purchase additional Voluntary Life and AD&D insurance for you and your dependents.

Disability

Disability insurance provides financial protection by offering benefits in the event of an inability to work due to injury or illness.

Basic Short-Term Disability (STD) covers 60% of your weekly earnings up to $1,200 per week and is 100% paid by Epsilon Systems. You have the option to supplement your coverage with Buy-Up STD which covers 70% of your weekly earnings up to $2,000.

Basic Long-Term Disability (LTD) covers 60% of your monthly earnings up to $6,000 per month and is 100% paid by Epsilon Systems. You have the option to supplement your coverage with Buy-Up LTD which covers 66.67% of your monthly earnings up to $10,000.

Voluntary Benefits

Employee-paid voluntary benefits are available to provide extra financial protection and peace of mind when unexpected events occur. These benefits are paid directly to you by Sun Life and you can use the money however you see fit. Each voluntary plan offers an annual $50 Wellness Screening benefit, access the form below to learn more.

Accident Insurance

Voluntary Accident Insurance provides cash benefits in the event you suffer a covered accident like fractures, burns, or emergency room visits. You can enroll yourself and your dependents in this coverage.

Hospital Indemnity Insurance

Voluntary Hospital Indemnity Insurance provides cash benefits in the event you have a hospital stay. You can choose between the Low and High plans with varying benefit amounts.

Critical Illness Insurance

Voluntary Critical Illness Insurance provides cash benefits in the event you are diagnosed with a covered illness like heart attack, stroke, or organ failure. You can choose specific coverage amounts for yourself and your dependents.
