Medical Plan Networks

Epsilon Systems provides two Kaiser Permanente medical options to help you meet your healthcare needs.

Kaiser HMO

The Kaiser HMO plan is a comprehensive medical plan that offers coverage for care provided through Kaiser facilities and providers only. It allows all your health care services to be coordinated by your primary care provider. The only out-of-network coverage this plan provides is for true emergencies or urgent care needs when you are outside of the service area.

Kaiser PPO

The Kaiser PPO plan is a comprehensive medical plan that offers coverage both in- and out-of-network. Out-of-network services will incur a higher cost. When searching for providers, you will have providers in Tiers 1, 2, or 3. Tier 1 providers will have the lowest costs.

Kaiser Permanente Medical Plans

Selecting the right medical plan is key to supporting your health and your family’s. Here’s a quick side-by-side comparison of the HMO and PPO, highlighting differences in costs, coverage, and flexibility to help you choose the plan that works best for you.

$0 Deductible HMO

Kaiser HMO Network

This plan has no deductibles. This plan will have lower premiums, but you must use Kaiser providers and facilities only.

$0 deductible PPO

Kaiser PPO Network

This plan has no deductibles for in-network care and a $100 individual and $300 family deductible for out-of-network care. This plan will have higher monthly premiums, but allows you to use providers outside of the Kaiser network.

Pharmacy Benefits

When you’re enrolled in a Kaiser medical plan, you have access to pharmacy coverage that provides prescription drugs and related supplies at discounted rates. The Kaiser HMO allows you to fill prescriptions at Kaiser pharmacies only. The Kaiser PPO allows you to fill at Kaiser and non-Kaiser pharmacies that are contracted with the plan, but you will pay the least when you use Kaiser pharmacies. Prescriptions filled at non-contracted pharmacies will not be covered.

TRICARE Supplements from Selman

TRICARE Supplement Insurance helps reduce out‑of‑pocket costs by covering expenses left after your primary TRICARE plan pays. It wraps around TRICARE Prime, Select, or Retired Reserve coverage and helps pay deductibles, copays, cost shares, and certain excess charges.

  • Uses the same doctors and pharmacies as TRICARE
  • Covers remaining eligible costs after TRICARE, up to the catastrophic cap

Kaiser Choose Healthy Online

Choose Healthy is a free program available to Kaiser medical plan members. Access discounts and resources for living well.

Kaiser Mobile App

Take your healthcare on the go with the Kaiser Mobile App. You can:

  • Schedule in-person and virtual care appointments

  • Email your providers

Virtual Care

Kaiser offers a variety of virtual care options for healthcare that fits your life. Whether you just need to send your provider a message, talk to a nurse or pharmacist, or a virtual video visit, Kaiser has alternative options available 24/7.

  • Virtual care by phone, video, or secure messaging for non-emergent conditions like colds, UTI, rashes, and more

  • 24/7 Nurse Advice Line: Call 1-808-432-7700 (Oahu) or 1-800-467-3011 (Neighboring Islands) to reach a nurse 24/7

  • After-Hours Care: Kaiser has after-hours clinics for those who need care outside of normal business hours at the same cost as a standard office visit

  • Travel Support: Kaiser has a dedicated travel line 1-951-268-3900 for members who may need care while traveling

Complementary Alternative Medicine (CAM)

Kaiser offers coverage for CAM services through American Specialty Health Systems. Members receive a set number of visits at a $20 copay per visit. Review your Evidence of Coverage for visit limits.

  • Chiropractic Care: Includes a $50 annual allowance toward chiropractic appliances or supports

  • Acupuncture

  • Massage Therapy

Sun Life – Dental PPO Plan

With the Dental PPO Plan through Sun Life, you have the flexibility to see any dentist, with greater savings when you stay in-network. Preventive services are included at no cost.

  • Oral exams and cleanings are covered 100%

  • $50 individual deductible and $150 family deductible

  • $2,000 calendar year maximum per member

  • Save the most when using in-network providers

Sun Life Vision Plan with the VSP Network

Epsilon Systems offers vision coverage through Sun Life with the VSP network to help you and your family maintain healthy eyes and save on the cost of exams, glasses, and contact lenses.

  • $10 copay for in-network eye exams and $25 copay for lenses every 12 months

  • $150 allowance + 20% off balance for frames every 24 months or

  • $150 allowance for contact lenses every 12 months

Flexible Spending Account (FSA)

A Flexible Spending Account (FSA) through Igoe lets you set aside pre-tax dollars for qualified expenses. FSA funds are use it or lose it and must be spent by the end of the year calendar year.

  • Healthcare FSA – covers eligible medical, dental, and vision expenses (available only with HMO or PPO plan enrollment).

  • Dependent Care FSA – covers eligible expenses like daycare (available to all employees regardless of medical plan).

Life and AD&D

Life and AD&D insurance provides financial protection by offering benefits in the event of death or dismemberment. Basic Life and AD&D is paid 100% by Epsilon and includes 1x your annual salary up to a maximum of $150,000. You may also purchase additional Voluntary Life and AD&D insurance for you and your dependents.

Disability

Disability insurance provides financial protection by offering benefits in the event of an inability to work due to injury or illness.

USAble’s Temporary Disability Insurance (TDI) covers 58% of your weekly earnings up to $837 per week and is 100% paid by Epsilon Systems.

Basic Long-Term Disability (LTD) covers 60% of your monthly earnings up to $6,000 per month and is 100% paid by Epsilon Systems. You have the option to supplement your coverage with Buy-Up LTD which covers 66.67% of your monthly earnings up to $10,000.

Voluntary Benefits

Employee-paid voluntary benefits are available to provide extra financial protection and peace of mind when unexpected events occur. These benefits are paid directly to you by Sun Life and you can use the money however you see fit. Each voluntary plan offers an annual $50 Wellness Screening benefit, access the form below to learn more.

Accident Insurance

Voluntary Accident Insurance provides cash benefits in the event you suffer a covered accident like fractures, burns, or emergency room visits. You can enroll yourself and your dependents in this coverage.

Hospital Indemnity Insurance

Voluntary Hospital Indemnity Insurance provides cash benefits in the event you have a hospital stay. You can choose between the Low and High plans with varying benefit amounts.

Critical Illness Insurance

Voluntary Critical Illness Insurance provides cash benefits in the event you are diagnosed with a covered illness like heart attack, stroke, or organ failure. You can choose specific coverage amounts for yourself and your dependents.
